Uninstall Conditional Field Values via Admin Center > Apps and integrations > Zendesk Support apps, then its cogwheel menu. Existing form conditions will stop working and require revision.
Nothing easier than uninstalling an App.
To uninstall the Conditional Field Values app, navigate to Admin Center > Apps and integrations > Zendesk Support apps. Locate the Conditional Field values App, click its cogwheel icon, and select Uninstall.
Note that by uninstalling the App, the conditions in your forms will stop working. You will need to revise your form(s).
